It was determined based on an investigation, that on January 26, 2024, at approximately 4:50 pm., while in the cafeteria, center staff allowed a child to engage in activities that were detrimental to the health and well-being of the five-year-old child in care. The staff members folded two tables in an upright position and tied a string from one table to the other table to act as a net for the children to play a game of balloon volleyball. The five- year-old child walked over to the table and stepped on the railing under the table causing it to fall on top of the child. The five-year-old child sustained several injuries that required professional medical treatment.
