During a Licensing Study conducted on March 14, 2024 ; It was determined based upon observation that one out of four plastic steering wheels were missing on the yellow metal bus. It was further determined that the blue plastic infant swing was cracked at the base of the swing causing sharp exposed edges. It was also determined that the top of the yellow slide was missing five out of six bolts that secured the slide to the top of the play structure causing a potential pinching hazard. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During a Monitoring Visit conducted on October 11, 2022 ; It was determined, based on review of records, that the driver or other designated person did not document in writing with a check or other mark/symbol the unloading of the vehicle for the following: •The unloading of 12 children at the end of afternoon routine transportation at the center from Flat Rock Elementary on September 30, 2022. •The unloading of three children at the end of afternoon routine transportation at the center from Fairington Elementary on September 30, 2022. •Two children were not marked absent for the unloading of the vehicle for afternoon routine transportation from Flat Rock Elementary to the Center on October 6, 2022. •The loading and unloading of three children was not marked for morning or afternoon transportation to/from the Center to/from Fairington Elementary on October 7, 2022. 2. During a Licensing Study conducted on April 27, 2022 ; It was determined based on a review of documentation there was not evidence of the following arrival and departure times on recent transportation trips: -April 11, 2022 - on an afternoon route from Farmington Elementary - no return or pickup was documented -April 11, 12, 13, 2022- on an afternoon route from Bowie Elementary - no return time was documented. 2. B. Rule 591-1-1-.36(7)(c )3. requires that the driver or other designated person document in writing the time of arrival and departure each time the vehicle departs from the Center, is loaded or unloaded at each school and when the vehicle returns to the Center; each time the vehicle departs from the Center, arrives at the location where any child is picked up or dropped off and when the vehicle returns to the Center; each time the vehicle leaves the Center, arrives at a field trip destination, leaves a field trip destination, and returns to the Center. During a Monitoring Visit conducted on October 11, 2022 ; It was determined, based on review of records, that the Center failed to document the following times: •The unload time for afternoon routine transportation from Fairington Elementary to the Center was not documented for September 30, 2022. •The times for departure from the center, load time at Flat Rock Elementary, and arrival to the center were not documented for afternoon routine transportation on October 6, 2022. Additionally, a staff purse was stored on a low shelf and a bottle of insect spray was stored in an unlocked cabinet under the sink. - two-year-old classroom- a bottle of sanitizer was stored on a low counter - three-year-old class bathrooms- one bottle of Comet cleaner and one bottle of Odoban disinfectant were stored in an unlocked bathroom cabinet -Pre-K classroom - push pins were observed on low walls accessible to children -cart with cleaning products was stored in a stall in the bathroom used by the children
