Classroom F/ Front Entrance -There were two plastic grocery bags at the front right in the children's cubbies. -There was a plastic grocery bag, a stapler, and a bottle of Comet cleaning spray in a storage cubby at the middle right of the classroom. -There was a bottle of Purell hand sanitizer and a pack of Huggies baby wipes in an unlocked storage closet at the right middle of the classroom.

During a Complaint Closure conducted on June 24, 2025 ; It was determined based on an investigation that staff failed to provide watchful oversight on May 6, 2025, when a 1-year-old child walked over to the children's cubby area and a plastic bin containing toys fell and hit the child on their head, causing a knot on the right side of their forehead. Staff were observed to be talking with each other and not monitoring the child.
