    During a Monitoring Visit conducted on February 6, 2023 ; It was determined based on the consultant's observation that three plugs in a power strip in the downstairs classroom were uncovered. It was further determined that plugs in the power strip near the television in the room next to the infant classroom were uncovered.

For example, toddlers shall not be permitted to swing in swings designed for School-age Children. The outdoor equipment shall be free of lead-based paint, sharp corners and shall be regularly maintained in such a way as to be free of rust and splinters that could pose significant safety hazard to the children. All equipment shall be arranged so as not to obstruct supervision of children.

    During a Monitoring Visit conducted on February 6, 2023 ; It was determined based on consultant's observation the playground equipment had the following hazards: - Rust was observed on the chains of all four swings. - S hooks on all swings were observed to be open. - Multiple sharp edges were observed on the arch. - Chipping paint was observed on the arch, the monkey bars, the four-seat see saw, and the yellow metal car. - Splintering wood was observed on the ramp leading to the front right playground. - The yellow/red and purple/pink riding cars had cracked wheels.
