591-1-1-.06(4) · Rule 591-1-1-.06(4) requires a Center first licensed after March 1, 1991, and Centers that remodel or add to existing plumbing facilities, to have the bathroom area fully enclosed and ventilated to the outside of the building with either an open screened window or functioning exhaust fan and duct system and requires Centers without fully enclosed bathrooms to ensure there is adequate ventilation to control odors and adequate sanitation measures to prevent the spread of contagious diseases.
During an Initial Licensing Study conducted on July 23, 2024 ; It was determined based on observation that the restrooms utilized for children in classroom A, classroom B, classroom D, classroom E, classroom F, and Pre-K (building two) did not provide exhaust fans to ventilate and sanitize restrooms after usage.

During an Initial Licensing Study conducted on July 23, 2024 ; It was determined based on observation that playground A had rust on the fence, top poles and fence panels and there was vegetation growing from the bottom of the fencing. Additionally, the front fencing measured at 3 feet 8 inches and the back fencing measured at 3 feet 9 inches and the front fencing had screws protruding on the gate and corner poles. It was further determined that Playground B was observed to have rust on the back fencing , the top poles had screws protruding on the fence gate.
