    During an investigation, it was determined that on September 3, 2024, center staff failed to protect a one-year-old child from unintentional harm when a staff member grabbed, pulled, and picked up the one-year-old child by the arm in a manner that would cause injury. Professional medical attention was required.

    During an investigation, it was determined that on September 3, 2024, a one-year-old child sustained a nursemaid’s elbow while in care. Center staff failed to submit a required report to the Department within 24 hours or the next work day as required after becoming aware that the one-year-old child had obtained professional medical attention. Center staff became aware that the child received professional medical attention on September 3, 2024, and did not report the incident as required.

Exposed tarp was also observed at the base of the wooden border surrounding the sliding structure. - Playground C-Rear Center - An unwrapped hose and a broken table were observed to be sitting on the front-left side of the playground. Equipment and furniture wrapped in plastic and in boxes and silver poles were sitting on the back-left side of the playground accessible to the children. Exposed tree roots were observed throughout the grounds of the playground. The PVC piping surrounding the sliding structure was separated. The exposed tree roots and the separated PVC created a potential tripping hazard. Broken tree branches were observed on the ground and hanging over the fence located on the back-left side of the playground.

The exposed tree roots and the separated PVC created a potential tripping hazard. Broken tree branches were observed on the ground and hanging over the fence located on the back-left side of the playground. Further, there was a deflated splash and slide accessible on the playground.

    During a Licensing Study conducted on June 5, 2024 ; It was determined based on observation that the following playground equipment did not have the required amount of resilient surface: - Playground B-1L - The fall zone surrounding the slide attached to the red, blue, yellow and brown sliding structure contained approximately one inch of resilient surface. - Playground C-Rear Center - The fall zone surrounding the slide attached to the red, green, blue, and yellow sliding structure contained approximately one inch of resilient surface. Three inches of resilient surface was required.

"Supervision" means that the appropriate number of Staff members are physically present in the area where children are being cared for and are providing watchful oversight to the children, volunteers and Students-in-Training. The persons supervising in the child care area must be alert, positioned to maximize their ability to hear and see the children at all times, and able to respond promptly to the needs and actions of the children being supervised, as well as the actions of the volunteers and Students-in-Training, and provide timely attention to the children's actions and needs.     During a Complaint Investigation & Monitoring Visit conducted on March 15, 2023 ; It was determined based on observation that an infant was sitting in a high chair eating during mealtime and two one-year- old children were sitting at a table alone and the staff member was not sitting within arm's length away from the child. The staff member was observed to be walking around the play area, rather than sitting closely to the children, as required.
