During a Complaint Investigation Follow Up conducted on August 29, 2022 ; It was determined based on observation that the sink in the two-year-old room was observed to be leaking.

During a Monitoring Visit conducted on March 23, 2023 ; It was determined based on observation that the sink in the two-year-old room was corrected. However, it was determined based on observation that in classroom D the toilet was stopped up and not working properly. Dead roaches were observed to be in classroom F in cabinet under diapering area. In the BR three-year-old classroom, the Consultant observed Clorox wipes and Febreeze spray in a container with sippy cups on a low open shelf, a metal scraper, two staplers, and a bottle of hand sanitizer were in the teacher area accessible to children, The trash can had no lid and food was placed in trash can in classroom B. Floors were observed to have dirt buildup throughout the center. Room H-Front right , had mold and dirt in cabinet under diapering area. The two-year old classroom was observed to have a tile that was broken at back door. Classroom G was observed to have three stained tiles in ceilings in the classroom, a stained sheet was observed to be in a crib.

During a Monitoring Visit conducted on March 23, 2023 ; It was determined based on observation that the back right fence had a large hole measuring 12 by 24 inches that would allow access to the adjoining property and parking lot. The latch from the enclosed sidewalk to the two year old playground was observed to be loose. The left rear playground fence was observed to be disconnected from the anchor pole. The fencing was observed to be less than 4 feet tall due to the the non anchored fence. Exhibit "A"
