During an investigation, it was determined that on March 9, 2026, at approximately 4:20 p.m., the center staff did not comply with a child's written plan for medical emergencies when a 1-year-old child sustained burn injuries. An unsecured infant bottle fell over from a countertop and hot formula spilled onto the 1-year-old child. The child was observed to have white and red skin that was inflamed and peeling on their left upper arm and torso. The child’s parent was contacted, and the parent was advised of the injury and that the child needed to receive medical attention. At 4:40 p.m., the parent arrived at the center and took the child to receive professional medical attention. The child was diagnosed with second-degree burns to their chest, left upper arm, and torso areas. The center did not seek prompt professional emergency medical treatment during the time of the incident. The center staff was aware of the severity of the child’s injury and did not contact professional emergency medical services.

During feeding time, a staff member warmed an infant bottle that contained formula. The staff member determined that the formula was too hot and placed the bottle on a countertop. Unaware that the bottle’s lid was not fully tightened, the teacher stepped away from the counter to intervene in an interaction between two other children. The teacher then heard the 1-year-old child scream and observed that the unsecured bottle had fallen, spilling hot formula onto the child who was on the floor near the counter. The child required professional medical attention and was diagnosed with second- degree burns to their chest, left upper arm, and torso areas.
