591-1-1-.25(3) · Rule 591-1-1-.25(3) requires the Center and surrounding premises to be kept clean, free of debris and in good repair. Hygienic measures such as, but not limited to, screened windows and proper waste disposal procedures shall be utilized to minimize the presence of rodents, flies, roaches and other vermin at the Center.
During an Incident Investigation Closure conducted on December 2, 2022 ; It was determined that on September 29, 2022, the bathroom in the two-year-old classroom was not maintained in a safe condition when a two-year-old child fell on the wet floor, resulting in a serious arm injury. After using the bathroom, the two-year-old child approached the sink for hand washing. As the child stepped on a stool in front of the sink, water on the floor caused the stool to slide out from under the child. The child landed on their arm. The child received professional medical treatment and was diagnosed with a serious arm injury. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During a Complaint Investigation & Licensing Study conducted on August 16, 2023 ; It was determined based on observation that the following items need to be repaired -Floor molds throughout the building were not in good repair and need to be repainted due to chipped paint. -Dividers in the Pre-K bathrooms were observed to have chipped areas of paint that children were observed to be actively peeling. - Rt Ft Classroom the drawer under the left changing table sink was observed to have chipped paint.
