Hygienic measures such as, but not limited to, screened windows and proper waste disposal procedures shall be utilized to minimize the presence of rodents, flies, roaches and other vermin at the Center.

    During a Licensing Study conducted on December 2, 2024 ; It was determined that paint was observed to be peeling throughout all classrooms on this date.

    During a Complaint Investigation Follow Up conducted on January 15, 2025 ; It was determined based on an observation that paint was observed to be peeling in the following classrooms: C-3L Toddlers had paint peeling on the white cabinet of the sink and D-1R had white peeling paint on several walls on this date. Due to the peeling white paint in the D-1R, red paint could been seen.

    During a Complaint Investigation Follow Up conducted on January 15, 2025 ; It was determined based on observation that the following hazards were located: --Playground-Right: Contained a green water hose attached to the drinking faucet on the concrete patio. --Playground-Middle Back: Contained a six inch by eight inch hole located in the grass near the swings, which is a tripping hazard. --Playground-Left Toddler: Contained several branches and tree limbs on the ground near the play equipment and accessible to children. One of the tree limbs measured 20 feet, six inches in length. All hazards were accessible to children.

    591-1-1-.32(7) · Rule 591-1-1-.32(7) requires that children be supervised at all times appropriate to the individual age, needs and capabilities of each child. Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities. "Supervision" means that the appropriate number of Staff members are physically present in the area where children are being cared for and are providing watchful oversight to the children, volunteers and Students-in-Training. The persons supervising in the child care area must be alert, positioned to maximize their ability to hear and see the children at all times, and able to respond promptly to the needs and actions of the children being supervised, as well as the actions of the volunteers and Students-in-Training, and provide timely attention to the children's actions and needs. Staff shall be attentive and participating with all children during mealtimes and shall be seated within an arm’s length away from children thirty-six (36) months of age and younger.

    During an investigation, it was determined that on January 3, 2025, staff failed to provide watchful oversight when a two-year-old child sustained an injury to their leg and staff were unaware of how the child was injured. The child’s injury was discovered when the child was picked up and the child was observed to be limping. The two-year-old child required professional medical attention for swelling in the left leg.

    During an investigation, it was determined that on January 3, 2025, the center inappropriately mixed children under three years of age in the same classroom as older children. There were three staff members present and 13 children in the classroom. There were five two-year-old children, three three-year-old children, and five school-aged children mixed in the same classroom for the entire day.

    During an investigation, it was determined that on January 3, 2025, a two-year-old child sustained an injury to their leg that required professional medical attention the following day, January 4, 2025. The Center staff was notified on January 3, 2025, that the child would be taken to a medical facility for treatment. The center staff did not report the incident within 24 hours as required.
