    During a Licensing Study conducted on March 6, 2023 ; It was determined based on observation an infant from Room 1 crawled out of the classroom unbeknownst to the two supervising staff members in the classroom. The child crawled to the front desk area where consultant, director and owner were standing and was unsupervised for approximately two to three minutes.
