During a Monitoring Visit conducted on July 17, 2025 ; It was determined based on observation that upon arrival at 9:15 a.m. and knocking on the entrance door, the consultant observed a total of five small children to exit classroom C, walk into the hallway and up the hallway towards the entrance door with no adult. Additionally, after knocking for approximately five minutes, the center director came to the door from the opposite hallway. Furthermore, the center director stated the children were supposed to follow the staff member when she walked out of the classroom and down the hall. Classroom C is out of direct sight of the left hallway and housed two 1-year-old children and six 2-year-old children.

591-1-1-.08(7) · Rule 591-1-1-.08(7) requires the Center to ensure that children are only released to authorized person(s), and that Center Staff take necessary steps to determine that any such person(s) presenting to pick up a child in care is authorized by the Parent(s) of the child and that person matches the identifying information provided by the Parent.
During an investigation it was determined that on February 17, 2023, center staff released a three-year-old child to a person not listed as an authorized pickup person on their enrollment application. The unauthorized person and the child left the premises. Staff interviews revealed that on February 17, 2023, a staff person failed to check the identification of an unauthorized person who was not listed on the child’s enrollment form. At approximately 8:45 a.m., an unauthorized person arrived at the center to pick up a five-month-old child but instead left with a three- year-old child. The staff person failed to check the children’s authorized pick-up list. The staff person allowed the child to leave with the unauthorized person. Upon the arrival of the authorized person to pick up the three-year-old child it was discovered that the wrong child was released to an unknown person. The police were called, and a missing child report was made. The staff member admitted the child was released to the person without checking the person’s identification or the child’s pickup list.

The center director walked to the two-year-old classroom and brought the one-year-old child to join the other children at the end of the hallway at 12:10 PM. The center director stated the one-year-old child had been in the two-year-old classroom alone with no watchful oversight and supervision for a few minutes, however, the consultant was present for twenty minutes and the child was in the classroom before the consultant arrived. During a Licensing Study conducted on January 19, 2022 ; It was determined based on observation and the center director's statement that a one-year-old child was left unsupervised while napping in the first classroom when entering the front door. Upon arrival, at 11:50 AM all staff and all other children were in the last classroom located at the end of the hallway. The center director walked to the two-year-old classroom and brought the one-year-old child to join the other children at the end of the hallway at 12:10 PM. The center director stated the one-year-old child had been in the two-year-old classroom alone with no watchful oversight and supervision for a few minutes, however, the consultant was present for twenty minutes and the child was in the classroom before the consultant arrived. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During a Complaint Closure conducted on April 21, 2022 ; It was determined based on the center director's statement, collateral contact information and pictures that a one-year-old child was returned home with a bruise resembling a bite mark on his cheek. The center director stated the child arrived at daycare on March 29, 2022 with no visible marks, was picked up by the mother that evening with a visible mark, and the teacher could not provide an explanation.
