During an investigation, it was determined that on August 5, 2024, the first check of the vehicle was not completed resulting in a six-year-old child being left unattended on the center vehicle for approximately forty minutes. The transportation vehicle arrived at the center at 3:02p.m. after picking up the school-age children from their elementary school. The required first check of the vehicle was not conducted by the staff member present on the vehicle before they exited the vehicle and entered the center. Based on the evidence collected during the investigation, it was determined that center staff failed to physically walk through the entire vehicle upon arrival at the center to ensure that there were no children left in the vehicle. The transportation checklist was completed and incorrectly marked indicating that a first check of the vehicle was conducted resulting in the six-year-old child being left unattended on the vehicle for 40 minutes.

During an investigation, it was determined that on August 5, 2024, a six-year-old child was left unattended on the center vehicle for approximately forty minutes. According to AccuWeather the temperature for that day was 90 degrees. Based on staff statements and transportation records, the child fell asleep on the vehicle. The child was discovered unattended on the locked vehicle by a staff member who was taking out the trash at the center. The staff member failed to provide adequate supervision and watchful oversight when the staff member was unaware that the six-year-old child was missing and left in the center vehicle for forty minutes.
