591-1-1-.32(7) · Rule 591-1-1-.32(7) requires that children shall be supervised at all times appropriate to the individual age, needs and capabilities of each child. Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities. "Supervision" means that the appropriate number of Staff members are physically present in the area where children are being cared for and are providing watchful oversight to the children, volunteers and Students-in-Training. The persons supervising in the child care area must be alert, positioned to maximize their ability to hear and see the children at all times, and able to respond promptly to the needs and actions of the children being supervised, as well as the actions of the volunteers and Students-in-Training, and provide timely attention to the children's actions and needs. During an investigation, it was determined that on March 31, 2026, adequate supervision was not provided when a 3-year-old child was left alone in a classroom and walked out of the center. At approximately 4 p.m., children lined up to go outside, however, the 3-year-old child did not exit the room with the group when the teacher led the other children out the exterior door. The child was alone in the classroom for approximately 3 minutes, then exited via the interior door and walked down the hallway to the lobby. After approximately 2 minutes, the child exited the center through the front entrance door and walked along the walkway adjacent to the parking lot. After approximately 1 minute of being outside the center, the child was noticed by another child’s parent, who returned the 3-year-old child to the building.

It was determined based on an investigation that on May 23, 2025, at approximately 3:30 p.m. on Playground C, center staff failed to properly intervene and redirect children from engaging in unsafe play when a five-year-old child was struck in the face by a wooden block that had been thrown into the air by another child. Professional medical care was obtained to repair a cut on the child's eyebrow.

It was determined based on an investigation that on May 23, 2025, children were observed tossing a wooden block across the basketball court on Playground C. A five-year-old child was struck by the block, which had been thrown into the air by another child. Center staff failed to ensure the children were playing with the wooden block in a safe manner and in accordance with its intended purpose. The five-year-old child obtained professional medical care to treat a cut to their eyebrow.
