During an Incident Investigation Closure conducted on January 31, 2023 ; It was determined based on photos and interviews that on October 10, 2022, the center did not remove hazardous equipment from the playground. A nine-year-old child had access to cracked and broken waffle blocks, the child jumped on the blocks and fell. The child required professional medical attention. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During an investigation, it was determined that on October 10, 2022, a nine-year-old child was allowed access to cracked and broken plastic waffle blocks while on the playground. The child was jumping on the blocks and fell, cutting their leg on a piece of broken plastic. The piece of plastic became embedded in the nine-year-old child’s leg and the child required professional medical attention due to the injury to their leg.
