    During an investigation, it was determined that on October 31, 2023, center staff failed to provide watchful oversight when two one-year-old children were observed to be near a small refrigerator that had a towel and electric bottle warmer on top of it. One child was pulling on the towel that hung over the side of the refrigerator and one staff member attempted to verbally redirect the child as they gathered their personal belongings to leave for the day, but did not attempt to position themself to respond promptly to the actions of the children. The child continued to pull on the towel and pulled the bottle warmer over the edge, spilling hot water on both of the children and burning their arms and legs. One child required professional medical attention as a result of the burns.

    During an investigation, it was determined that on October 31, 2023, an electric bottle warmer was accessible to children in care when it was placed on top of a small refrigerator with a towel underneath the bottle warmer. Two children were observed to be near the refrigerator and one child pulled on the towel, pulling the bottle warmer off of the edge of the refrigerator, spilling hot water. As a result, hot water spilled onto the children’s arms and legs. One child required professional medical attention.

    During an investigation, it was determined that on October 31, 2023, staff were using an electric bottle warmer in the infant classroom that the manufacturer rated for household use only and was not intended for commercial use. Staff placed the bottle warmer on top of a small refrigerator with a towel underneath it. Two one-year-old children were playing near the refrigerator and one child pulled on the towel, causing the bottle warmer to fall and spill hot water onto the children burning their arms and legs. One child required medical attention as a result of the incident.

    During an investigation, it was determined that on October 31, 2023, two one-year-old children were observed to have red marks and burns from a bottle warmer that fell off the top of a small refrigerator. One of the children had pulled the bottle warmer off the refrigerator and spilled the hot water onto the children. One child sustained burns on their arms and legs, however their parents were not notified of the incident until the following day when they asked center management about blisters observed on their child’s arms and legs. Staff notified center management at the time of the incident but neither center management or the staff reported the incident to the parents at the time of the incident or by the end of the day. The child received medical attention as a result of the incident.
