During a Complaint Investigation & Licensing Study conducted on September 18, 2024 ; It was determined based on observation that the ramp leading to the sidewalk was not flush with the ground posing a tripping hazard. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During a Complaint Investigation & Licensing Study conducted on September 18, 2024 ; It was determined based on a review of records that 17 of 17 children being transported did not have emergency medical forms on the vehicle.

During a Complaint Investigation & Licensing Study conducted on September 18, 2024 ; It was determined based on a review of records the loading and unloading of children was not accounted for during the week of August 5, 2024 through August 9, 2024 in that on August 5, 2024 three children were not marked as loaded/unloaded during afternoon transportation from Michelle Obama Stem Academy. It was further determined that on August 7, 2024 and August 8, 2024 three children were not marked as loaded/unloaded during morning transportation to Michelle Obama Stem Academy, and one child was not marked as loaded/unloaded from Rocky Creek Elementary. In addition, on August 9, 2024 the center marked that transportation was completed during the morning and afternoon transportation to and from Michelle Obama Stem Academy and Rocky Creek Elementary, however; no children were marked as loaded/unloaded. On September 3, 2024 six children were not marked as loaded/unloaded during morning transportation to Michelle Obama Stem Academy and three children were not marked as loaded/unloaded during afternoon transportation from Michelle Obama Stem Academy.

It was further determined that on August 6, August 8, and August 9, 2024 the second check was not conducted during afternoon transportation from Rocky Creek Elementary. Additionally, during the week of September 2, 2024 the second check was not conducted during morning and afternoon transportation to and from Michelle Obama Stem Academy from September 3, 2024 through September 6, 2024.
