During a Complaint Closure conducted on March 20, 2023, it was determined based on investigative findings, that on February 3, 2023, while on the playground, a six-year-old child jumped on a wooden landscaping border and fell. The child landed with one knee on the wooden border, resulting in an injury to their leg. The wooden border was not in good repair. The wood was splintered, uneven, and had multiple protruding screws. The child received professional medical attention as a result of the incident.

During a Complaint Investigation and Follow Up Visit conducted on May 18, 2022, it was determined based on observation that the following items were in need of repair: -Wood on the wooden structure of the back right playground was observed to be chipped. -Gray riding toy on the front playground was missing the seat.

During an incident investigation/monitoring visit conducted on March 7, 2022, it was determined based on observation that a staff member was observed to pinch a two-year-old child on the face while on the center playground. No marks or bruises were observed. In addition, a staff member was observed to scream at the child on the playground in a harsh tone stating, "Stop it. Stop it". When the class returned to the classroom, the staff member bent down to the child's face and screamed, "I don't have time for this".

During an investigation on October 13, 2022, it was determined based on investigative findings that on undetermined dates between September 2022 and October 2022, center staff confined 2 two-year-old children to high chair booster seats on the playground for approximately 5 minutes. Also, on other various dates in September 2022 and October 2022, a four-year-old child with special needs was confined to a high chair booster seat as a form of discipline for approximately 15 minutes. Video footage revealed that 2 two-year-old children were confined in a high chair, crying, and laying on their side on the ground on the playground and in the classroom.

During a complaint investigation and follow up visit conducted on October 13, 2022, it was determined based observation that two staff members in Room B used inappropriate discipline during nap time. Staff were heard threatening to call children's parents, take treats away, and take away toys if they did not go to sleep. Staff were also heard to use harsh tones in telling the children to lay down.
