During an Incident Investigation Closure conducted on July 17, 2025 ; It was determined based on investigation that on June 10, 2025, the staff failed to immediately notify the parent when a two-year- old child fell and was injured and required professional medical treatment. It was also determined that the parent was not notified until the end of the day when the child was observed by the staff with symptoms of moderate discomfort.

During a Complaint Closure conducted on July 31, 2025 ; It was determined that on June 30, 2025 at approximately 1:30 p.m, a staff member observed a 5-year-old child with an injury to the forehead resulting from a fall. The staff did not notify the parent of the child's injury.

During an Incident Investigation Closure conducted on April 15, 2025 ; It was determined that on February 12, 2025, children were not adequately supervised when a two-year-old child sustained a laceration below the right eyebrow. During free play, the child took a couple steps and stumbled, hitting their head on the edge of the back window. The child sat on the floor under the back window, bleeding from a cut under the right eyebrow. The two staff persons in the classroom were positioned at the diaper changing table. The child sat on the floor bleeding for approximately three minutes and 50 seconds, unnoticed by staff persons. The child received professional medical treatment and the laceration was closed with stitches.

During a Complaint Closure conducted on July 31, 2025 ; It was determined based on an investigation that adequate supervision was not provided on June 30, 2025 when a 5-year-old child sustained an injury to the forehead. At approximately 1:30 p.m., a staff member observed a mark on the child's forehead and was unaware of how the child sustained the injury. The child revealed to the staff member that they had fallen. The child later revealed to the parent that they had fallen on the playground and hit their head on a pole. Exhibit "A"

During an investigation, it was determined that on February 12, 2025, two staff members were unaware that a two- year-old child had tripped and fallen into the window sill, in the back of the classroom, sustaining an injury to the right eyebrow. One staff member had their back to the window and the other one was positioned at the changing table, not able to see the area where the incident occurred. The child sat injured for approximately three minutes and fifty seconds before being checked on by a staff member. A staff member in an adjoining classroom tapped on the window to let staff know the child was injured. The injury required professional medical attention.
