During an investigation, it was determined that on January 17, 2023, the center did not anchor a piece of climbing equipment when a slide and tunnel combination piece of equipment was not anchored as required. While on the playground children were able to tip the piece of equipment over onto a three-year-old child. The child sustained an injury to the arm that required professional medical attention.
