    591-1-1-.10(5) · Rule 591-1-1-.10(5) requires that Center Staff shall not leave infants or children unattended while being diapered or having their clothes changed on the diaper changing surface.

    During an investigation, it was determined that on July 23, 2025, at 2:40 p.m., a 1-year-old child was left unattended on a diaper changing table for four seconds while in the 1RT 2nd Hall Rt-Side Classroom. The teacher turned away from the child and towards the sink when the child fell forward and onto the floor. The child required professional medical attention and was diagnosed with an injury to the forehead.
