During a Complaint Investigation Closure conducted on March 3, 2026, it was determined based on an investigation that on December 2, 2025, two staff members were unaware of how a 1-year-old child sustained an injury to the forehead. One staff member was changing another child’s diaper, while the other staff was entering information into the parent app when the incident occurred. Staff stated once they were aware the child had fallen and hit their head, first aid was administered. The injury required professional medical attention.
