During an Incident Investigation Closure conducted on August 28, 2025 ; It was determined based on an investigation that a 2-year-old child sustained an injury to the collarbone after falling from the green slide on Playground Main B-2R on August 7, 2025, at approximately 9:45 a.m. There was a staff-to- child ratio of 2:13 on that date, that consisted of two 1-year-old children and eleven 2-year-old children. Neither staff witnessed the incident. Medical attention was obtained. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During a Complaint Closure conducted on March 9, 2026 ; It was determined based on an investigation that on February 10, 2026, Center Staff failed to provide watchful oversight when a 6-year-old child was left alone on the playground for approximately 15 minutes. Exhibit "A"

During an investigation, it was determined that on August 7, 2025, staff failed to provide watchful oversight when a 2-year-old child sustained an injury while on the playground. The child fell from the side of a slide and landed on the ground. Two staff members were present but did not see the child fall. After the fall, the child began to cry and walked over to one of the staff members, who thought that the child might be upset and holding their throat area due to either being hot or having a sore throat. After the child was picked up by a parent, professional medical attention was received, and the child was diagnosed with a broken collarbone.
