In Room A, a can of Lysol was on top of a gray wooden shelf, and the gray tall cabinet near the exit door was unlocked with hazardous materials inside, also accessible to children.

Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities. "Supervision" means that the appropriate number of Staff members are physically present in the area where children are being cared for and are providing watchful oversight to the children, volunteers and Students-in-Training. The persons supervising in the child care area must be alert, positioned to maximize their ability to hear and see the children at all times, and able to respond promptly to the needs and actions of the children being supervised, as well as the actions of the volunteers and Students-in-Training, and provide timely attention to the children's actions and needs. During a Complaint Investigation & Monitoring Visit conducted on September 24, 2025 ; It was determined based on observation that when the consultant returned from the playground, the staff in Room A was not present, leaving the children unsupervised. In Room B, the staff member entered and exited the kitchen multiple times, also leaving the children unsupervised. Additionally, the staff member in Room B placed an infant in a crib located in an isolated area with a door but no walls. In Room C, the staff member was observed walking around the room during mealtime rather than being seated at the table as required.
