During a Monitoring Visit conducted on August 7, 2024 ; It was determined based upon observation that the following fencing hazards were present on this date: PG- Left Playground -The fencing on the back left side was bent at the top rail. -The middle of the left side fence top pole was rusted. -The chain link fencing on the left side was rusted. -The bottom of the fencing was not connected causing a five inch gap on middle of the fencing on the right side of the playground. PG- Back Right Playground -The chain link fencing was detached from the top pole in the middle section of the back fence. -There was a ten inch by three inch gap from the ground to the fence at the middle of the right side fence. PG-Front Playground -The front chain link fence measured three feet and five and one half inches when four feet was required. -The top pole was rusted at the middle of the front fence. -The bottom of the chain link fence was not properly connected causing a five inch gap on the bottom of the gate doors on the top right side of the playground. -There was a seven and one half inch gap at the bottom of the gate door fencing on the back right side of the playground. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

The manipulative shelf in the middle of the classroom had one adult stapler accessible on the top middle shelf. A staff purse was in an unlocked cubby on the right side of the classroom. 591-1-1-1.32(2) · Rule 591-1-1-1.32(2) requires that for Centers with a licensed capacity of 19 or more children. The Staff: $499.00 child ratios for a mixed-age group shall be based on the age of the youngest group of children that includes more than twenty (20%) of the total number of children in the mixed-age group.
During an Incident Investigation Closure conducted on July 1, 2021, it was determined that on May 17, 2021, at 10:00 a.m., a three-year-old child sustained an injury to the head while on the playground. There were two staff members supervising 22 three-and four-year-old children. One of the staff members escorted a child inside to change the child’s pull-up. Staff interviews revealed that the staff members were aware that leaving the playground would leave the remaining staff member out of ratio. Staff interviews revealed that while one staff member was inside the center, a child approached the staff member that was on the playground stating that the three-year-old child fell and hit their head on the play structure. The staff member did not observe the child fall. The staff members received a written disciplinary action and signed the document acknowledging being out of ratio. Professional medical attention was obtained. There was one staff member supervising 16 three-year-old children and 5 four-year-old children on the playground, and the required ratio was 1:15. Based on the number and ages of the children present, two staff were required.
