During a complaint investigation and follow-up visit conducted on September 9, 2025, it was determined based on an investigation that on August 28, 2025, a 1-year-old child fell from a piece of playground equipment, which was fifteen inches from the ground, and sustained an injury to the right arm. The piece of playground equipment had an intended use for children one and a half years of age to six years of age. When the incident occurred, the child was one year and four months old. Professional medical attention was required for a fractured arm.

During a licensing study conducted on July 10, 2025, it was determined based on observation that two of the six tricycles on the middle playground were missing pedals. In addition, one tricycle was missing the handle grips and three of tricycles needed to be repaired to be in working condition.

Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities. During an investigation, it was determined that on July 24, 2025, staff members failed to provide watchful oversight when in the gym at approximately 8:12 a.m. a 5-year-old child fell and hit the back of their head on the gym floor. Staff were unaware that the child was involved in a spinning game. Staff members did not become aware of the incident until the injured child, and another child alerted them. Professional medical attention was obtained.

During an investigation, it was determined that on July 24, 2025, center staff failed to submit a required report to the Department about an incident within 24 hours or the next work day after becoming aware that a child had received professional medical care to treat a head injury sustained while in care at the center. Center staff were informed on July 25, 2025, that medical treatment was obtained. The center did not report the incident to the department until July 29, 2025.

During an investigation, it was determined that on June 1, 2023, children were not adequately supervised when a one-year-old child’s thumb was closed in the hinge side of a classroom door. At approximately 9:40 a.m., there was one staff person with seven children. The staff person was in the process of changing diapers. The staff person needed to get diapers for one child out of their bag hanging on a hook right outside the classroom door. The staff person opened the door and reached their arm around the corner to retrieve the backpack. The staff’s body remained in the classroom. As the staff person was closing the door, the one-year-old child cried out. The staff person was not aware that child was behind the door. The staff person observed the child’s thumb was bleeding and bruised. Management was notified and first aid was rendered. The child’s parents were notified. The child received professional medical treatment.

During an investigation, it was determined that on March 9, 2023, at approximately 3:00 p.m., a five-year-old child injured their head on the restroom door that required professional medical attention. The school-age children were standing outside the boys’ and girls’ restroom. Two of the children started horse playing. Another child attempted to break the two children up by pushing the five-year-old child causing the child to hit their head against the girls’ bathroom door. At the time of the horse playing the staff person was in the girls' bathroom and did not provide watchful oversight to the children. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.
