Main 2nd Left -A broom at the back right of the classroom in front of the handwashing sinks. Main 1st Left -A clear plastic bag with diapers inside in a child's cubby. Main 3rd Left -A broom at the front right corner of the classroom. -A pack of Huggies wipes on a low shelf next to the changing table.

During a Complaint Investigation & Licensing Study conducted on August 21, 2025 ; It was determined based on observation that the following potential hazards were accessible to children on this date: Building II Classroom A -An unlocked storage closet at the front left of the room with mops, fans, and electrical cords. Classroom 2nd Right -A plastic grocery bag at the front left of the room under a table near the handwashing sink. -A plastic grocery bag at the back left of the room on a shelving unit. Storage Passageway between Classroom A, 1st Left and classroom B, 2nd Left -A vacuum cleaner, a broom, a mop, and a dust pan at the left middle of the room. -A plastic grocery bag at the front left of the room. Classroom B, 2nd Left -A plastic grocery bag on a shelf at the front right of the room. Classroom D, 1st Right -A pack of Pampers baby wipes inside the children's cubby. -A clear plastic bag with diapers in a box under a table located at the front right of the room. Classroom E, 1st Center -There were three plastic grocery bags in an unlocked cabinet underneath the sink. Classroom G, 2nd Center -A plastic grocery bag inside a child's cubby.

During a Monitoring Visit conducted on April 14, 2025 ; It was determined based on observation that the diaper changing table in Classroom C Main 3rd Left was not clean and free of debris as required when there was dirt and food crumbs underneath the changing pad. It was further determined that a bug was crawling on the changing surface on this date. It was additionally determined that the middle toilet stall in the boy's bathroom in Building II was observed to not have a stall door as required.

During a Complaint Investigation & Licensing Study conducted on August 21, 2025 ; It was determined based on observation that the following items were not in good repair on this date: Building II Classroom A -A bug crawling on the wall near the storage closet located at the front left of the room. -Outer wall of the first stall in the boys bathroom had rust at the back bottom edge. -A section of the wall at the bottom back right of the boys bathroom, inside the right stall, with a hole revealing a wooden beam and drywall/plaster. -Left inner wall of the first stall in the girls bathroom had rust at the back bottom edge. Classroom B, 2nd Left -Lid of the toilet tank in the girls bathroom had a large side-to-side crack with sharp jagged edges. Classroom A, 1st Left -Spider webs above the door and around the light fixture inside the boys bathroom. -Exhaust fans inside the bathrooms were dusty. -Return vent at the front right of the room near the bathrooms was dusty with unsecured, protruding edges separating from the wall. Classroom G, 2nd Center -Return vent at the front right of the room was extremely dusty.

During a Complaint Investigation & Licensing Study conducted on August 21, 2025 ; It was determined based on observation that upon the Consultant's arrival at classroom B, 2nd Left, Center staff permitted a 3-year-old child to leave the classroom and independently walk through a storage passageway towards the entry door of classroom A, 1st Left. Center Staff watched and encouraged the child to open the classroom door, but the child could not open the door without assistance. It was further determined that Center Staff in classroom 2nd Right left the classroom for an undetermined amount of time when they were seen re-entering the classroom through the back door of classroom 3rd Center, leaving six children unsupervised. It was additionally determined that Center Staff in classroom 2nd Right left six children unsupervised for an undetermined amount of time when the Consultant observed the Staff member standing inside the neighboring classroom G, 2nd Center. Additionally, the carpet had been taped down and was lifting in some areas posing a potential tripping hazard. Main D, 1st Right - There was a crack in the white floor tile next to the windowsill that needed to be repaired.

Main B, 2nd Left - During a diaper change a student was observed to pick up and play with diapers and a bag of baby wipes that were stored on the changing table.

The red top of the bin containing the waffle blocks in the block center was observed to be cracked and broken with sharp edges and accessible to children in the Block Center. The broom and dust pan were observed accessible to children in the corner of the classroom. Room A, 1st Left ±Plastic bags were observed accessible to children in cubbies. A bottle of hand sanitizer, a container of Oxivir TB wipes, and a tape dispenser with a serrated edge were observed accessible to children on a low table on this date.

During a Licensing Study conducted on April 7, 2022 ; It was determined based on observation that the following hazards were observed: BLD II A - One of the toilets in the classroom was not working and had a sign on the door saying out of order. This toilet needed to be repaired. Main 2nd right - The plaster on the lower left side of the wall in the girl¶s restroom had fallen onto the floor and had not been cleaned up. Main 3rd Center - There was rust in the sink in the classroom. The duct tape on the bottom of the wall was coming off the wall, and posed a potential tripping hazard, and the black border on the wall next to the back door had peeled of the wall which also posed a potential tripping hazard. Additionally, the back of the two white storage units in the classroom had come loose and posed a potential pinching hazard. Main A 1st Left - On the wooden area next to the sink that had been painted red, the paint was peeling and needed to be repaired. Main B, 2nd Left - There was peeling paint on the back of the wall and the border near to the back exit door had become detached and needed to be repaired.

The light shield in the Girls¶Bathroom was observed to be missing on this date. One of two handwashing sinks was observed to not be in working order on this date. Room E, 1st Center ±A baseboard located to the right of the back door to the classroom was observed to be detached from the wall. 2nd Right Classroom - The left side of the counter of the handwashing sink near the wall was observed to be detached from the counter on this date.

The annual ten (10) clock hours of training shall be chosen from the following fields: child development, including discipline, guidance, nutrition, injury control and safety; health, including sanitation, disease control, cleanliness, detection and disposition of illness; child abuse and neglect, including identification and reporting, and meeting the needs of abused and/or neglected children; and business related topics, including parental communication, recordkeeping, etc.; provided however that such business related training shall be limited to no more than two (2) of the required ten (10) clock hours of training. Records of completion of such training shall be maintained, as required by these rules. During a Licensing Study conducted on April 7, 2022 ; It was determined based on review of records that staff member #5, and staff member #8, had not completed the annual ten (10) hours of diverse training for the 2021 calendar year as required. 5.
