During an Incident Investigation Closure conducted on March 7, 2025 ; It was determined based on an investigation that on January 31, 2025, center staff did not provide watchful oversight when they were not positioned where they could see all the children in their care all of the time. A six-year-old child sustained a left elbow contusion and wrist fracture after being pushed to the ground by another child. Professional medical was obtained. During an investigation, it was determined that on January 31, 2025, a six-year-old child sustained an arm injury when the child was lined up with their class, in the center hallway, when the child was pushed by another child causing the child to fall. The staff member was in another classroom retrieving supplies when the incident occurred and did not witness the incident. Professional medical attention was obtained.

During an investigation, it was determined that on November 29, 2021, children were not adequately supervised when a three-year-old child with a known allergy to fish, had an allergic reaction when the child sat at a table in front of a plate containing fish. The plate was removed before the child ate any food. Between three and six minutes later, the child’s eyes were red and watery and bottom lip was swollen. The child was brought to management and the child’s parent was notified. The parent arrived shortly thereafter and administered Benadryl. The child received professional medical treatment.

591-1-1-.15(9) · Rule 591-1-1-.15(9) requires that when a child requires a modified diet for medical reasons, a written statement from a medical authority shall be on file. When a child requires a modified diet for religious reasons, a written statement to that effect from the child’s Parent(s) shall be on file. 591-1-1-.29(3) · Rule 591-1-1-.29(3) requires the Director or designated person-in-charge to report or cause to be reported to the Department within twenty-four (24) hours or the next work day: any death of a child while in the care of the Center; any serious illness or injury requiring hospitalization or professional medical attention other than first aid of a child while in the care of the Center; any situation when a child in care becomes missing, such as, but not limited to, a child who is left on a vehicle, a child who leaves the building, playground, or property, or a child who is left behind on any trip; any fire; any structural disaster; any emergency situation that requires temporarily relocating children; and any time the program’s operating status changes (i.e., open to closed or temporarily closed and temporarily closed to open).
