During an investigation, of a self-reported incident, it was determined that on October 3, 2023, at 11:21 am center staff failed to ensure that the furniture in the gym was free from hazardous conditions when a three-year- old child tripped and fell into the metal bleachers that were not properly covered. Staff statements indicated that the child was chasing after a ball during free play in the gym. While chasing the ball the child tripped, fell, and hit their forehead on the edge of a metal bleacher. The sharp edge of the bleacher did not have a protective cover. Professional medical attention was required.
