During an Incident Investigation Closure conducted on October 20, 2023 ; It was determined based on investigative findings a self-reported incident revealed that on September 21, 2023, at 7:36 a.m. watchful oversight was not being provided to a one-year-old child who was in a highchair, when the staff member turned away from the child after removing the tray and didn't realize that the safety straps were not being used. The child fell out of the high chair and sustained scratches and a bruise.

During a Complaint Closure conducted on December 19, 2023 ; It was determined based on investigative findings that, on November 29, 2023, center staff left a five-year-old child unattended on the playground when a child holding the door let the door close before the five-year-old child entered the classroom. The five-year-old child opened the door to enter the classroom.

During a Complaint Closure conducted on August 26, 2024 ; It was determined that two staff did not provided watchful oversight when a child was observed hitting another child on the face, hitting the child on the head, pulling his hair and snatching items from his hands. The child was observed crying as the result of the physical altercation. The two staff were observed in the classroom, however the staff were not alert to promptly intervene on behalf of the child.
