During an investigation, it was determined that on September 9, 2025, center staff failed to provide watchful oversight when staff members were unaware that a 4-year-old child had left the classroom and center building unattended. The 4-year-old child was found wandering the center parking lot before being returned to the center two minutes later by a parent of another enrolled child. Professional medical attention was not obtained.

During an investigation, it was determined that on September 9, 2025, a 4-year-old child left the center without staff knowledge and was found in the parking lot by another parent. Center staff did not notify the child’s parent at the time of the incident as required. The child’s parent arrived to pick the child up shortly after the incident and was not notified of the incident until after they left the center for the day.

During an investigation, it was determined that on February 14, 2025, at approximately 10:00 a.m., there were one staff member and ten children in the classroom. The center staff inappropriately disciplined a 2-year-old child while in the C-3L classroom. The center staff roughly grabbed the victim child and two children by their limbs multiple times in efforts to redirect the children and forcing the children to sleep. The center staff failed to protect the children from harm and injury as required by the Department. The victim child cried and showed signs of distress while at the center. The child sustained multiple dark purplish bruises on their upper arms. The child did not require professional medical attention. As required by the Department, the center staff failed to refrain from physical force and rough handling the children.
