During a Complaint Investigation & Licensing Study conducted on April 9, 2021 ; It was determined based on observation that five bottles on the counter top were not labeled and five bottles in the refrigerator were not labeled as required. Additionally, staff stated that two bottles were left out of the fridge all day per a parent's request and not refrigerated as required.

Additionally, the thermometer in the refrigerator was not in working order preventing the staff from knowing if the bottles were stored at the appropriate temperature.
