During an incident investigation, it was determined that on February 27, 2025, a three-year-old child walked in front of a moving swing, and was struck in the face while on the playground. There was one staff member supervising thirteen afterschool children on the playground. At the time of the incident, the staff member was assisting another child on the playground. The children were observed to be running throughout the playground. The three-year-old child walked in front of a moving swing, was struck and kicked in the face. The child fell to the ground and remained there briefly, then walked to the opposite side of the playground, to the staff member to obtain aid from the staff member. Staff failed to actively supervise and provide watchful oversight when they were not aware the child was injured or how the child sustained the injury. The child required professional medical attention as a result of the incident and was diagnosed with a lip laceration and dental trauma.

During an investigation, it was determined that on July 17, 2024, when a staff member was changing the diaper of an enrolled child, a one-year-old child pulled down the bottle warmer, that was on the corner of the counter in the classroom. The staff was not aware that the child was near the bottle warmer and of the child’s injury, until they cried out. The child received professional medical attention.
