During a Licensing Study conducted on August 25, 2025 ; It was determined based on observation that the following potential hazards were accessible to children on this date: Classroom 4th R- G -There was an unlocked cabinet at the front right with an aerosol can of Lysol and a plastic grocery bag. School-Age Front Right Hallway -There was a window squeegee with a long handle leaning against the wall located at the back right corner. Classroom 1st L- A -There was an unlocked closet at the front right with a large white trash bag and a black trash bag. Classroom 2nd L- D -There was a plastic grocery bag on the teacher table at the back middle of the room. Classroom 3rd L- E -There was a pack of baby wipes under the diaper changing table. -There was a clear plastic diaper packaging with diapers under the diaper changing table. Classroom 4th R- F -There was a broom and a dust pan at the front right of the classroom. -There were three plastic grocery bags in an unlocked cabinet at the back middle of the room. -There was an aerosol can of Lysol Disinfectant Spray, two bottles of Clorox Hand Sanitizer, and an aerosol can of Glade spray on top of a storage cabinet at the back middle of the room.

During a Complaint Closure conducted on June 18, 2025 ; It was determined, based on an investigation, that on April 28, 2025, at 5:45 p.m., center staff did not provide watchful oversight when a 3-year-old child wandered from the premises for approximately four minutes and was found by a stranger. The 3- year-old child followed a teacher from another class to the entrance door, and the teacher verbally directed the child to return to the playground. Staff watched the child walk to the corner of the building but did not ensure the child returned safely to the playground. The child walked away from the premises and was found by a stranger near traffic approximately 206 feet from the center. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

The staff member was walking around and assisting other children that were preparing for nap on this date. Exhibit "A"

During an investigation, it was determined that on April 28, 2025, a 3-year-old child was redirected by a staff member, to return to the playground after he followed a teacher that was not his, but the child did not and left the center premises. There were four children and three staff members on the playground when the incident occurred. Staff stated once they redirected the child, they saw him go around the corner to the playground but not actually return to the playground. When the child’s mother arrived at the center to pick up, she was flagged down by a stranger who had found her child. The child was found approximately 206 feet from the center without socks or shoes on. The child was missing for approximately four minutes.

Classroom C - A plastic grocery bag was observed in the cubby accessible to children in care. - Eucerin cream was observed in a gray bookbag in the red cubby accessible to children in care. - Diaper wipes were observed in children's bookbags in the red cubby accessible to children in care. Classroom D - A plastic grocery bag was observed in a flower bookbag in the white cubby accessible to children in care. - Diaper wipes observed in the black and white striped, dinosaur, and green and yellow bags in the cubby accessible to children in care. Hallway restroom - Lysol spray stored on the paper towel dispenser accessible to children in care.
