During a POI Follow Up conducted on September 3, 2024 ; It was determined based on observation that approximately two inches of rubber mulch was observed at the fire pole and climbing pole on playground A, when six inches was required. It was further determined that approximately two inches of rubber mulch was observed around the swings, when six inches was required on playground A.

During a LS POI Follow Up conducted on August 14, 2025 ; It was determined based on observation that approximately two inches of rubber mulch was observed at the fire pole and climbing pole on playground A, when six inches was required. It was further determined that approximately three inches of rubber mulch was observed around the swings, when six inches was required on playground A. It was also determined that approximately one inches of rubber mulch was observed around the yellow slide, when six inches was required on playground A.

During an Incident Investigation Closure conducted on May 6, 2025 ; It was determined based on an investigation that on March 4, 2025, staff failed to provide adequate supervision when they were unaware that a one-year-old child was injured after another child threw a book and hit the child in the eye. The staff member was not alert or positioned to maximize their ability to hear and see the children at all times and able to respond promptly to the needs and actions of the children being supervised. The one-year-old child was treated at a medical facility and diagnosed with an abrasion to the right cornea. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During a LS POI Follow Up conducted on August 14, 2025 ; It was determined based on observation that a 4-year-old child walked out of classroom I:1st Right without adult supervision for approximately two minutes into the front hallway until retrieved by a staff member and taken to classroom G. During an investigation it was determined that on March 4, 2025, at approximately 3:30 p.m., center staff failed to provide adequate supervision when they were unaware that a one-year-old child was injured. A child in the classroom threw a book hitting the one-year-old child in right eye. The staff member was not alert or positioned to maximize their ability to hear and see the children at all times and able to respond promptly to the needs and actions of the children being supervised. The one-year-old sustained an injury to their right eye which required professional medical treatment.

It was determined based on an investigation that on July 26, 2021, individual attention was not provided to a one-year-old child as required after an altercation between two one-year-old children. A one-year-old child was observed to walk up to another one-year-old child, then grab the second child by the neck. The two children fell to the floor and rolled around. The first child was observed to bite the second child on the face. After the staff person intervened the first child was observed to grab the second child by the neck again. The one-year-old child obtained multiple scratch marks and a bite mark as a result of the altercation. Center staff did not offer comfort to the child, but instead kept them at arm’s length while examining their injuries and later placed the child alone at a table.
