During an investigation, it was determined that on March 13, 2026, at approximately 4:49 p.m. on the playground, a 4-year-old child sustained an injury when a blue metal equipment cart struck their head. During outdoor play, a staff member heard the child cry out and noticed that the child had a bleeding cut on their head. The injured child and another child had been near a blue metal equipment cart that had tipped over; however, it was unknown as to how the cart tipped, as staff members did not observe the incident. Professional medical attention was obtained, and the child was diagnosed with a scalp laceration. The staff members were not positioned to see all children as required.

During a Licensing Study conducted on March 25, 2026, it was determined based on observation that children entered the front door of the program, walked down the hallway, and entered into classroom D (MR) without adult supervision. It was further determined that a child walked up the hallway for dismissal without adult supervision for approximately one minute.
