    During a Licensing Study conducted on February 20, 2026 ; It was determined based on observation that the following playground hazards were accessible to children on this date: Playground A- Infants, 1's & 2's: -There were five exposed iron stakes around the wooden barrier. -There were thorny vines at the front left chain link fence. Playground B- 3's & Up -There were three exposed sharp edges of concrete located at the back left of the playground. -There were a plastic grocery bag and a mop located near the entrance of the playground.

    During a Monitoring Visit conducted on March 5, 2025 ; It was determined based on observation that the following hazards were accessible to children on this date: Classroom C: The mounted televisions had an exposed cord that was connected to a surge protector with exposed outlets. Classroom E: The mounted televisions had an exposed cord and a stapler on top of a desk in the back corner of the classroom. Bathroom that connects classrooms B, C and D: 2 plungers, a broom, and a dust pan were in a corner across from stacked containers. Classroom A: Chipped paint with exposed sheet rock in multiple areas at the bottom of the window sills. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.
