    591-1-1-.25(3) · Rule 591-1-1-.25(3) requires the Center and surrounding premises to be kept clean, free of debris and in good repair. Hygienic measures such as, but not limited to, screened windows and proper waste disposal procedures shall be utilized to minimize the presence of rodents, flies, roaches and other vermin at the Center.

    During a Complaint Investigation Follow Up conducted on January 11, 2023 ; It was determined based on observation that the following hazards were observed: 1st Right - Unraveling purple tape at the back of the carpet next to the sink area. 2nd Right - Unraveling tape in the block area near the music shelf. 1st Left - Missing wall border observed in the back left side of the classroom next to the blue cabinet. - A broken floor tile observed next to the playground exit door. 3rd Left - Missing ceiling tile at the door entrance.

    During a LS POI Follow Up conducted on April 21, 2023 ; It was determined based on observation that the following hazards were observed: 2nd Right - Unraveling tape on the carpet in the block area near the music shelf. 1st Left - A damaged ceiling tile above the second toilet in the restroom. - A damaged ceiling tile at the entrance of the restroom door. - Peeling paint observed on the right wall under the grass poster.
