During a Monitoring Visit conducted on July 22, 2022 ; It was determined based on observation that, in the A 1st Left classroom there was a bottle of bleach solution, Clorox wipes, and a Clorox aerosol can on the changing table, accessible to children.

During a Licensing Study conducted on March 24, 2023 ; It was determined based on observation that the following potential hazards were observed: In the B 2nd L classroom, there were plastic bags and wet wipes that said keep out of reach of children on the changing table accessible to children. In the C 3rd Left classroom, there were plastic bags hanging on the children¶s pegs that said keep out of reach of children accessible to children. In the D Entrance classroom, there were plastic bags on the paint easel and plastic bags containing items purchased for the party that said keep out of reach of children accessible to children. In the E Rear Left classroom, there were two bottles of Lysol spray and Lysol wipes stored on a low shelf at a level accessible to the children. In the F Rear Right classroom, there was a black trash bag, white trash bags under the children¶s cubbies, and white trash bags in the red storage unit next to the cots, accessible to children.

During a Complaint Investigation Follow Up conducted on April 21, 2023 ; It was determined based on observation that plastic bags that say warning to avoid danger of suffocation, keep this plastic bag away from babies and children, were hanging on children's pegs at low level, on the back wall behind the diaper change station accessible to children in the A 1st L infant classroom.

Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities. "Supervision" means that the appropriate number of Staff members are physically present in the area where children are being cared for and are providing watchful oversight to the children, volunteers and Students-in-Training. The persons supervising in the child care area must be alert, positioned to maximize their ability to hear and see the children at all times, and able to respond promptly to the needs and actions of the children being supervised, as well as the actions of the volunteers and Students-in-Training, and provide timely attention to the children's actions and needs. Staff shall be attentive and participating with all children during mealtimes and shall be seated within an arm¶s length away from children thirty- six (36) months of age and younger.
During a Complaint Closure conducted on June 21, 2022 ; It was determined that on April 29, 2022, center staff failed to provide adequate supervision when staff members in the two-year-old classroom did not see and prevent a child from biting a two-year-old child on their face.

During a Complaint Closure conducted on December 21, 2022 ; It was determined based on investigative findings that a six-year-old child sustained a burn on their neck, from a hot glue gun, while in attendance at the afterschool program. A Staff member was using the hot glue gun when some of the glue fell from the glue gun onto the back of the six-year-old child's neck. Staff at the center cleaned the area and applied some antibacterial cream. No professional medical attention was obtained as the parent applied over the counter treatment. It was further revealed that the parent received an apology from the center and the Staff member involved in this incident was terminated on the day that the incident occurred.

During a Complaint Investigation Follow Up conducted on April 21, 2023 ; It was determined based on observation that Staff were not providing watchful oversight to four infants and three one-year-old children in the A 1st Left infant classroom, as both Staff members were out of the classroom at the same time leaving the children unsupervised. One Staff member was covering in another classroom at the back of the building, the other Staff member had left the classroom and gone into the classroom next door, closing the adjoining classroom door, so the children could not be seen at all times. The children were unsupervised for an unknown period of time, but when the Staff member and consultant stepped into the classroom, one infant was in a crib, the other six children were crying and sitting on the floor close to the adjoining classroom door. During a Complaint Investigation Follow Up conducted on June 29, 2021 ; It was determined based on observation that the center housed 20 three-year-old children and seven two-year-old children in room E. A staff:child ratio of 2:27 was observed when a staff:child ratio of 3:27 was required.

During a Complaint Closure conducted on July 16, 2021 ; It was determined based on staff statements and documentation that on May 3, 2021, Center staff housed a total of 33 children and two staff persons in room E. It was further determined that five of the 33 were two-year-old children and the remainder were three and four-year-old children.
