During a Complaint Investigation Follow Up conducted on November 30, 2023 ; It was determined based on observation that the fall zones under the yellow slide and climbing equipment did not have adequate resilient surfacing beneath the equipment on this date. There was zero inches of measurable wood chips in the fall zone of the equipment, when six inches was required.

Fencing material shall not present a hazard to children and shall be maintained so as to prevent children from leaving the playground area by any means other than through an approved access route. Fence gates shall be kept closed except when persons are entering or exiting the area.

During a Monitoring Visit conducted on November 1, 2023 ; It was determined based on observations a gap measuring approximately six inches was observed on the front right side gate facing the front parking lot area on this date.

All equipment shall be arranged so as not to obstruct supervision of children.

During a Complaint Investigation Follow Up conducted on November 30, 2023 ; It was determined based on observation that the following equipment hazards were observed: Playground A - There was chipping and flaking paint on the small wood gazebo/playhouse. - There was chipping paint on the red tunnel. - The white PVC pipe was cracked and had a hole. Playground B - There was chipping and flaking paint on two of two race cars. - There was chipping paint on the yellow tunnel caboose.

In addition, there was excess swing chains on four of four swings on this playground and bolts exposed on deteriorating landscape in between the two trees. - The green outdoor carpet was rolled back on the infant/toddler playground, creating a possible tripping hazard.

In order for the file to be complete, the file shall contain the following: child¶s name, birth date, sex, address, living arrangement, name of school if applicable; names of both Parents, home and work addresses, and home and work telephone numbers; name(s) and addresses of the person(s) to whom the child may be released including address, telephone numbers, relationship to child and to Parent(s), and other identifying information; name(s) and telephone number(s) of person(s) to contact in emergencies when the Parent cannot be reached; name and telephone number of the child¶s primary source of health care; and a statement regarding known allergies, physical problems, mental health disorders, intellectual disabilities or developmental disabilities which limit the child¶s participation in the program.

During a Complaint Closure conducted on November 30, 2023 ; It was determined based on a review of records one child's enrollment application was not updated to include the complete information of the release to person, including the telephone number and relationship to the child.
