Also, the first front section was observed to be detached. Further, the fence at the back right corner was observed to be 3 feet, 6 inches and the front right corner of the fence was observed to be 3 feet, 7 inches.

During a Licensing Study conducted on December 18, 2025 ; It was determined based on observation that the bottom of the fence was not secured. Further, there was a gap measuring approximately 6 inches at the bottom of the fence between Playground Large and Playground Middle, loose and protruding wires on the gate between playground Large and the walkway to classroom 1L.

Additionally, the red wooden porch outside of the Room 3R on the Playground Middle had a spindle missing and a child went through the gap to other side causing entrapment.

591-1-1-.11(2) · Rule 591-1-1-.11(2) requires that Personnel not: physically or sexually abuse a child or engage or permit others to engage in sexually overt conduct in the presence of any child enrolled in the Center; inflict corporal/physical punishment upon a child; shake, jerk, pinch or handle a child roughly; verbally abuse or humiliate a child which includes, but is not limited to, the use of threats, profanity or belittling remarks about a child or his family; isolate a child in a dark room, closet or unsupervised area; use mechanical or physical restraints or devices to discipline children; use medication to discipline or control children's behavior without written medical authorization issued by a licensed professional and given with the parent's written consent; restrict unreasonably a child from going to the bathroom; punish toileting accidents; force-feed a child or withhold feeding a child regularly scheduled meals and/or snacks; force or withhold naps; allow children to discipline or humiliate other children; or confine a child for disciplinary purposes to a swing, highchair, infant carrier, walker or jump seat.
During an investigation, it was determined that on August 1, 2023, at approximately 3:15 p.m., a staff member was observed on video footage to roughly handle a five-year-old child as a form of discipline. Video footage showed the staff member grabbing the child by their arms abruptly, dragging them across a bench, and then throwing the child to the ground from the staff member’s waist height. The staff member was then observed to lift the child up by the torso and carry them toward the building. The child sustained bruises to their left arm. No medical attention was obtained. The staff member was arrested as a result of the incident. 2. Rule 591-1-1.31(13) prohibits Center Staff from committing any criminal act, as defined under Georgia law, in the presence of any child enrolled in the Center and requires Center staff to comply with all applicable laws and regulations. During an investigation, it was determined that on August 1, 2023, at approximately 3:15 p.m., a staff member was observed on video footage to roughly handle a five-year-old child as a form of discipline. During an investigation, it was determined that on September 15, 2022, a five-year-old child was left unsupervised on a vehicle for approximately one hour and twenty-five minutes. Staff stated on September 15, 2022, at 3:35 p.m., proper first and second checks were not conducted, and a five-year-old child was left alone on the vehicle. Staff stated the vehicle was parked, staff went inside, and the child was lying down, asleep on the seat. Staff stated at approximately 5:00 p.m. the child woke up and began to honk the vehicle's horn, alerting a parent who found the child. The child was unsupervised for approximately one hour and twenty-five minutes. There was no injury sustained to the child. No professional medical attention was sought due to this incident.

Staff were unaware that the child was left on the vehicle until the child woke up and began honking the horn and was found by another parent arriving at the facility. There was no injury sustained by the child and no professional medical attention was sought due to the incident.
