During an investigation, it was determined that on August 14, 2025, at approximately 10:00 a.m., staff failed to intervene when a 1-year-old child placed their finger in the hinge of the door, resulting in an injury that required professional medical attention. Staff observed another parent coming into the classroom door and observed the child walking over to the door that the parent opened. Staff did not intervene to ensure that the child’s fingers were not in the door before it closed.
