During a MV POI Follow Up conducted on March 12, 2026 ; It was determined based on observation that there was a white mini refrigerator sitting on a white cube shelf in the back right corner of Classroom 4-2nd Right, not secured, wobbly, and accessible to children.

During an Incident Investigation & Follow Up conducted on January 9, 2026 ; It was determined based on observation that there was a white mini refrigerator on a shelf not secured, wobbly, and accessible to children in the Ft. Hall Jonah's Way 4- 2nd Right classroom.

591-1-1-.32(7) · Rule 591-1-1-.32(7) requires that children be supervised at all times appropriate to the individual age, needs and capabilities of each child. Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities. "Supervision" means that the appropriate number of Staff members are physically present in the area where children are being cared for and are providing watchful oversight to the children, volunteers and Students-in-Training. The persons supervising in the child care area must be alert, positioned to maximize their ability to hear and see the children at all times, and able to respond promptly to the needs and actions of the children being supervised, as well as the actions of the volunteers and Students-in-Training, and provide timely attention to the children's actions and needs. Staff shall be attentive and participating with all children during mealtimes and shall be seated within an arm¶s length away from children thirty- six (36) months of age and younger.
During an Incident Investigation Closure conducted on January 27, 2026 ; It was determined based on an investigation that on December 18, 2025, a 2-year-old child's right finger was closed in the hinge side of the door when another child exited the restroom in the New Wing 10-2nd Right classroom. Two staff members were present but were not positioned to see the incident. The child received professional medical attention for a broken finger. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During an investigation, it was determined that on December 18, 2025, at approximately 10:40 a.m. adequate supervision was not provided when a 2-year-old child's right finger was closed in the hinge side of a restroom door. The child was positioned by the door hinge with their fingers placed in the gap between the door and the door frame when another child exited the restroom and closed the door. Two staff members were present in the classroom but were not positioned to see all of the children and did not see the incident occur. The 2-year-old child received professional medical attention for a broken finger.
