During a Complaint Investigation & Licensing Study conducted on May 25, 2023 ; It was determined based on observation that the yellow and red slide located on a preschool playground was cracked posing a potential hazard to children.

During a Complaint Investigation & Monitoring Visit conducted on August 24, 2023 ; It was determined based on observation that the red slide located on a preschool playground was cracked posing a potential hazard to children. No preventative measures were observed to block children from using the slide.

During a Complaint Closure conducted on September 8, 2023 ; It was determined based on an investigation that the red slide on the preschool playground had a crack spanning across the width of the slide, posing a hazard to children. The persons supervising in the child care area must be alert, positioned to maximize their ability to hear and see the children at all times, and able to respond promptly to the needs and actions of the children being supervised, as well as the actions of the volunteers and Students-in-Training, and provide timely attention to the children's actions and needs. During a Complaint Investigation Follow Up conducted on August 29, 2022 ; It was determined based on observation that a four-year-old child left the playground, walked inside of a classroom, left out of the classroom to go to the restroom alone in the hallway. After the four-year-old was finished in the restroom, the child left the restroom in the hallway, entered a classroom and went back outside without staff supervision. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During a complaint investigation, it was determined that on September 27, 2022, a one-year-old child was observed to have a bruise and swelling to the right side of the forehead with scratches to the right side of the eye and cheek. Staff stated no one observed the child to fall down or how the child sustained the injury. The one-year-old child was taken for professional medical care and was diagnosed with a contusion to the forehead and facial abrasions. The director and staff members did not have any knowledge of how or when the child sustained the injury and no report was provided to the parent.

During a complaint investigation follow-up, it was determined that on August 29, 2022, based on observation that a four-year-old child left the playground, walked inside of a classroom, left out of the classroom to go to the restroom alone in the hallway. After the four year old was finished in the restroom, the child left the restroom in the hallway, entered a classroom and went back outside without staff supervision.

591-1-1-.29(3) · Rule 591-1-1-.29(3) requires the Director or designated person-in-charge to report or cause to be reported to the Department within twenty-four (24) hours or the next work day: any death of a child while in the care of the Center; any serious illness or injury requiring hospitalization or professional medical attention other than first aid of a child while in the care of the Center; any situation when a child in care becomes missing, such as, but not limited to, a child who is left on a vehicle, a child who leaves the building, playground, or property, or a child who is left behind on any trip; any fire; any structural disaster; any emergency situation that requires temporarily relocating children; and any time the program’s operating status changes i.e., open to closed or temporarily closed and temporarily closed to open.
During an investigation, it was determined that the center was notified by parents on September 28, 2022, that professional medical care was obtained for a one-year-old child who sustained an injury at the center. There was no required report submitted to the department as required. Staff stated they were notified of the child's injury on September 27, 2022, and that the parent was taking the child for professional medical care. Staff further stated on September 28, 2022, the parent provided follow up from the physician regarding the injury sustained. According to evidence obtained, the staff indicated that they did not submit a required report to the department as required.

During an investigation it was determined that on June 17, 2022, center staff allowed a two-year-old child to run in the classroom resulting in the child sustaining a laceration to the chin when the child fell and hit their chin on the floor. The staff member did not physically intervene to stop the child from running. Professional medical attention was obtained, and the two-year-old child received five stitches to the chin.

During an investigation it was determined that on June 17, 2022, center staff only allowed a parent access to the lobby when a two-year-old tripped, fell, and hit their chin on the floor. Professional medical attention was required. The parent was not allowed access to all child care areas of the Center as required.

A staff member cared for three one-year-old children, five two-year-old children, three three-year-old children, and four four-year-old children alone on June 17, 2022, when a two-year-old child tripped, fell, and hit their chin. Professional medical attention was required. A ratio of 2:15 was required.

During an investigation it was determined that children under three years of age were not housed in a separate area from older children after the first hour of the day during early morning arrival. The center opens at 6:00 a.m. and children of all ages were still housed together at 7:20 a.m. when a two-year-old child tripped, fell, and hit their chin.
