During a Complaint Closure Visit conducted on June 5, 2023, it was determined based on investigative findings that center staff repeatedly yelled at and threatened physical forms of discipline to infant and one-year-old children in care on May 4, 2023.

During a Complaint Investigation Follow Up conducted on December 21, 2022 ; It was determined based on observation that the resilient surfacing around the climbing equipment and the fall zone around the swings measured between approximately one and two inches on playground PG/C/Right. Three inches of resilient surfacing is required around the climbing equipment and six inches of resilient surfacing is required in the fall zone around the swings.

Further, one side of the double gate between the school age and toddler playgrounds was bent, creating a gap between the two sides which measured approximately five inches. The gate also contained a gap measuring approximately four and a half inches. The fence on the left playground behind the climbing structure contained protruding nails.
