    591-1-1-.12(4) · Rule 591-1-1-.12(4) requires that equipment and furniture shall be secured if it is of a weight or mass that could cause injury from tipping, falling, or being pulled or pushed over. Potentially unstable equipment and furniture that might injure a child if not secured include, but are not limited to, televisions, chests of drawers, bookcases, shelving, cabinets and fish tanks. Examples of items not required to be secured include, but are not limited to, child-sized tables and chairs, rocking chairs, and cribs.

    During an investigation, it was determined that on June 5, 2025, at approximately 9:15 a.m., a 2-year-old child sustained a hand injury caused by a piece of dramatic play furniture falling in the classroom. At the time of the incident, four staff members were supervising nine 2-year-old children. Staff Members A, B, and C were transitioning groups of children to and from the playground, while Staff Member D was transitioning children into the hallway for a group activity. The 2-year-old child climbed onto the piece of dramatic play furniture, which then tipped over and landed on the child’s hand. Staff Member D immediately picked up the child, took them to the office, and administered first aid. Staff applied ice, and continued to monitor the child. The child was diagnosed with a hand injury that required professional medical treatment.
