The driver or other designated staff person shall document in writing with a different mark/symbol to account for each child listed on the passenger transportation checklist who was not present on the vehicle for any reason. During a Complaint Investigation & Monitoring Visit conducted on December 6, 2022 ; It was determined based on a review of records that there was no load or unload documented for the following: Middle Ridge Elementary: -One child on the afternoon route on November 29, 2022. -Two children on the afternoon route on December 1, 2022. -Two children on the afternoon route on December 2, 2022. South Salem Elementary: - One child on the afternoon route on November 17, 2022.

During an Incident Investigation Closure conducted on February 1, 2023 ; It was determined based on an investigation, that on April 20, 2022, center staff left a six-year-old child on a vehicle, for an undetermined amount of time between one and ten minutes. Staff were unaware that the child had been left on the vehicle until the child exited the vehicle, walked to the center door and rang the doorbell. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

It was further determined that children transitioned to the playground and in and out the building twice without supervision. Exhibit "A"

During an incident investigation, it was determined that on April 20, 2022, at approximately 7:35 a.m., watchful oversight was not provided when a six-year-old child was left on a center vehicle unattended and unsupervised for an undetermined amount of time between one and ten minutes. The child exited the vehicle and was found at the door of the facility by another staff member.
