    During a Complaint Investigation Follow Up conducted on May 14, 2025 ; It was determined based on observation that there were fourteen children in classroom B- 2nd Left when the maximum licensed capacity for that classroom was ten. Exhibit "A"

    During a Licensing Study conducted on March 18, 2025 ; It was determined based on observation that there were twelve children in classroom B- 2nd Left where the maximum usable space allowed for up to ten children. Additionally, there were thirteen children in classroom A- 1st Left where the maximum usable space allowed for up to twelve children.

    During a Licensing Study conducted on March 18, 2025 ; It was determined that the following hazardous items were accessible to children: Pre-K Building Right: -There was a broom and dust pan by the entry door to the classroom. -There was a plugged in laminator machine on a low table by the entry door inside the classroom. -There was a mop bucket with water less than half inch of water in it near the entrance to the kitchen. -There was a vacuum near the kitchen entrance door. -There was a plunger and a scrub brush in the left restroom. -There was a broken window within children's reach near the circle time area. Classroom C- Back Center: -There was a vacuum cleaner behind the door of the classroom. Classroom E- 1st Right: -There were stacked chairs in the classroom accessible to children.

B - 2nd Left: -There was a roll of duct tape with sharp rigid edges on a shelf next to the entrance. E- 1st Right: -There was a pack of wipes on a shelf next to the entrance. -There was a box of gloves behind the diaper changing table. -There was a pack of gloves behind the diaper changing table. -There was a tube of diaper rash cream behind the diaper changing table.
