Three bolts on the plastic barrier were sticking up, five pieces of trash were on the ground, and six black bags were scattered around the area.     During a Licensing Study conducted on February 4, 2026 ; It was determined based on observation in the 1st Left classroom that a staff member was not providing active supervision as the staff did not look up, or verbally redirect children, or circulate around the room when a child was found by the Consultant crouched next to the furthest toilet in the bathroom. The staff member was also unaware of a child in the block area pulling down their pants and unable to supervise children on the other side of the L shaped classroom while assisting children in the bathroom. Additionally, the staff member in the Back Right classroom stood between two classrooms to supervise children in both their classroom and going to a bathroom across the Middle classroom. It was also determined that staff in the 1st Right classroom were not within arm's length of two infants during meal time. Exhibit "A"

    During a Complaint Investigation & Monitoring Visit conducted on August 29, 2023 ; It was determined after a review of documents that the following second checks were not indicated on the transportation checklist: -One child that was transported on the morning route from the center to Peeks Chapel Elementary on August 18, 2023. -Two children that were transported on the afternoon route from Peeks Chapel Elementary on August 25, 2023. -Three children that were transported on the afternoon route from C.J. Hicks Elementary on August 23, 2023. -Six children that were transported on the afternoon route from Hightower Elementary on August 23, 2023.

    During an Incident Investigation/Licensing Study conducted on November 15, 2022 ; It was determined based on observation that the staff:child ratio was 1:16 in the 2nd Right classroom when there were twelve children who were one-year-old and four children who were two-years-old, which required a staff:child ratio of 2:16. It was further determined based on observation that the staff:child ratio was 1:16 in the 1st Left classroom when there were thirteen children who were three-years-old and three children who were four-years-old, which required a staff:child ratio of 2:16.
