    591-1-1-.36(7)(b) · Rule 591-1-1-.36(7)(b) requires that an emergency medical information record be maintained in the vehicle for each child being transported. The emergency medical information record for each child shall include a listing of the child's full name, date of birth, allergies, special medical needs and conditions, current prescribed medications that the child is required to take on a daily basis for a chronic condition, the name and telephone number of the child's doctor, the local medical facility that the Center uses in the area where the Center is located and the telephone numbers where the Parents can be reached.

    During a Licensing Study conducted on December 14, 2022 ; It was determined based on a review of records that the following were missing: -Four of eighteen children being transported from Barksdale Elementary were missing doctors names and phone numbers. -Three of five children being transported from Honey Creek Elementary were missing doctors names and phone numbers. -Seven of twenty-two children being transported from Oak Hill Elementary were missing doctors names and phone numbers.

    During a Licensing Study conducted on December 14, 2022 ; It was determined based on a review of records that there was no second check documented on the following: -Afternoon route from Honey Creek Elementary to the center on November 30, 2022. -Afternoon route from Barksdale Elementary to the center on December 6, 2022. -Afternoon route from Barksdale Elementary to the center on December 7, 2022. -Morning route from the center to Barksdale Elementary on November 30, 2022.
